    I understand that, depending on the nature of my volunteer assignment, Imam Ahmad Bin Hambal Islamic Center may find it necessary to obtain a Driver’s License Record and/or Criminal Background Check. I hereby give my consent for the Center to request my Driver’s License Record, Criminal History Report, and/or Wanted Information Report as needed.

    By providing my consent, I release and discharge Imam Ahmad Bin Hambal Islamic Center, along with its employees, agents, and representatives, from any and all claims or liabilities that may arise as a result of the requested background reports.
